FAMILY SUPPORT COORDINATOR

Up to £13.84 per hour (dependant on experience)

1 x Full Time role (37.5 hrs per week) OR 2 x Part Time roles (20 hrs per week) available.

This position is funded with thanks to Stoke on Trent City Council – Family Matters Programme, and is on a fixed term 12 month basis.

KEY DUTIES & RESPONSIBILITIES

  • Carry out outreach work, in family homes and communities.
  • Adapt their approach according to the family circumstances/needs.
  • Builds self-esteem, skills, relationships and aspirations.
  • Understand and where appropriate apply, locality and partnership solutions.
  • Develop a Single-Family Plan (owned and developed with the family)
  • Be empathetic but with clear boundaries.
  • Effective help is provided early to prevent needs escalating.
  • Support will be practical and provide sufficient hands-on support to address family issues.
  • A whole-family approach, supporting families to address a range of issues in the context of the whole family, not one individual.
  • Working collaboratively with other organisations and groups in and across city areas as a connector, joining up services around the family to tackle the root causes of their issues, for example, linking up with debt advice; fuel advice; housing and/or domestic violence services.
  • Developing the families’ ability to self-manage and use their extended support network, so that they feel empowered to manage challenges and take much greater control over their own health and wellbeing.
  • Helping families to develop parenting capacity and understand when and where to seek support.
  • Encouraging families to make positive choices in relation to their wellbeing.
  • Utilising digital and other sources of information and helping to develop the quality of that information through feedback and other input.
  • Knowledge and experience of using Liquid Logic System desirable but full training and support provided if not.

Due to the nature of the organisation, there may be additional duties and responsibilities required in addition to these above.
